Identification Agreement	Identification: Students and faculty must present a valid SFSU Identification card to use our services.
Online Records Access Agreement	Online Records Access: Student records are accessible on our website. Students are responsible to periodically check their record for any material checked out from this facility.
Overdues Agreement	Overdues: Overdue materials result in suspension of lab privileges. An additional notice to your instructor and department chair may be issued.
Student Checkout Agreement	Student Checkout: Most material are circulated for one week with up to three possible renewals. Renewals can be issued over the phone during office hours, although items in high demand may not be renewed. Five items per day may be checked out. All materials issued may be returned to the Cahill Lab counter with staff assistance during open hours or in the drop off box during closed hours. NOTE: Students will be charged for damaged for unreturned materials.
Test Collection Agreement	Test Collection: College of Education students with prior faculty authorization can use test materials. Students who need to purchase any test forms/protocols should first pay a laboratory fee and present a cashier's receipt to staff before receiving the forms. All fees may vary--please ask staff for details. NOTE: Fees cannot be reimbursed.
Two Hour Checkout Agreement	Two Hour Checkout: Education test score keys, special reference books and field studies are used on the premises for two hours with proper student identification. Student identification cards must be left at the counter while materials are checked-out. All items must be returned to the circulation counter before the time limit expires.
